IEP data collection sheets: match the sheet to the goal, not the other way around
Most searches for “IEP data collection sheets” end in a stack of cute downloads that never get filled in. The problem usually isn’t the sheet — it’s that the sheet doesn’t match the goal. Every measurable IEP goal names a measurement method, and the IEP itself must describe how progress toward each annual goal will be measured (34 CFR §300.320(a)(3)). The data sheet is just that sentence turned into a form. Pick the sheet the goal’s criterion demands — frequency, accuracy, duration, prompt level, task analysis, intervals, or work samples — and the data starts collecting itself.

The goal snippets above are invented samples. The test for any sheet: can you compute the exact number in the goal’s criterion from its columns? A tally sheet cannot prove “80% accuracy,” and a percent-correct sheet cannot prove “fewer than 2 call-outs per class.” If the sheet can’t produce the criterion, either the sheet is wrong or — just as often — the goal’s measurement language was never really measurable, which is a goal-writing problem, not a data problem.
Prompt codes: the five letters that save your data
Independence goals live or die on prompt-level data. A simple hierarchy code — I independent, V verbal, G gestural, P partial physical, F full physical — turns “he did it with help” into a data point. Print the code key on the sheet itself; the person collecting is often a paraprofessional covering three students at once, and a sheet that needs explaining is a sheet that comes back blank. A system that survives a real caseload
- Header = goal verbatim. Copy the goal’s criterion onto the top of the sheet. Substitutes, paras, and related-service providers then collect the same thing you do.
- One sheet per goal for trial data; one clipboard sheet per student for the day. The clipboard layer catches quick tallies in the moment; transfer to per-goal sheets weekly, not “someday.”
- Schedule collection like instruction. A goal measured “weekly via teacher-collected data” needs a standing slot in the week, or it will be reconstructed from memory in the week grades are due — the pattern that collapses in a file review.
- Start from the baseline. The first rows of every sheet should be the baseline data the goal was written from, so the trend line has an anchor.
